Commercially prepared mac 'n cheese noted in the hot holding unit at 115 degrees and not reheated to 140 degrees minimum within two hours (correction-product voluntarily discarded). Poly cutting board at small prep top cooler noted in poor repair with heavy staining and cut marks. Some floor areas not clean-primarily under three bay sink, prep cooler in front service aisle and rack in rear kitchen next to dishwasher. Ambient temperature of walk in cooler 41.7 degrees. Ambient temperature of low service line coolers 42 and 43 degrees. Sliced Tomatoes 42 degrees. Ground beef patties 42 degrees in low cooler.
